An annotated bibliography?

For my college cl I have to write an annotated bibliography about a nursing related subject. I was sick the day it was handed out so I have no clue what to do? We have to note five diff websites and write about the same subject off of the five diff websites. They gave us a list of websites but idk how to write a bibliography and idk what to write about. So pretty much what I am asking is how do I write a college level bibliography and how can I write about the same thing five times? And how do I site the sources in MLA format and what is the format of an annoted bibliography? Also what nursing Topic should i pick? any suggestions would be appreciated. Please please please help me. This is due tomorrow. Examples would b nice to see
